Thank you for your interest in signing my candidate petition.  Your support is what will make this campaign a success!  Here are some guidelines for filling out the petition:

  1. Petitions may only be filled out by registered voters that live in Escambia County District 5
  2. Petitions MUST be filled out completely and signed by the voter
  3. The voter should fill out the following blanks:
    1. Print name on the first line of the petition as it is printed on their voter registration card
    2. Date of birth or voter registration number as found on their voter registration card
    3. Residence Address
    4. City
    5. County
    6. State
    7. Zip
    8. Date Signed (THIS MUST BE FILLED IN BY THE VOTER)
